This page is only meant to provide honest information to the public about any End of Life Doula's that Earthside feels needs further education and accountability for unacceptable actions against a client or family's well-being.
Earthside Holistic trains their Doula's to hold the highest form of respect and accountability to their clients. Our Doula's sign a contractual agreement to adhere to Earthside Holistic's Code of Ethics in order to hold our certification. In signing, they agree they will do no harm and agree to be added to this list should they commit ethical violations. Should any Doula be found to be in violation of these Codes, Earthside reserves the right to revoke certification and add their name, general offense and accountability actions to this probation list. We do this in effort to keep our clients and community informed and safe from those who would harm them. Earthside offers a 6 month probationary period to any Doula who has violated the Code of Ethics as well as offer additional training and mentorship, because we believe in second chances (especially for unintentional offenses). However, Earthside will not issue or reinstate certification to repeat offenders.

End of Life Doula Agreement
I agree that as an End of Life Doula, I will conduct my behavior in a manner that will honor this growing profession, the client and family, and the end of life process.
I agree that as an End of Life Doula, I will uphold the highest level of personal and professional integrity and implement the standard of care with truth and accountability. Further, I agree to consistently remain reliable and dependable to my clients.
I agree that my responsibilities as an End of Life Doula lie first with my clients. I agree to uphold the rights and choices of my clients within the boundaries of state and federal laws.
I agree that as an End of Life Doula, I will respect the State and Federal mandated privacy acts. Further, I will take applicable precautions when posting anything on the internet.
I agree that as an End of Life Doula, I will maintain respectful interaction with all end of life professionals. ie: Doctors, Nurses, Social Workers etc.
I agree that as an End of Life Doula, I will endorse and encourage the growth and expansion of the End of Life Doula profession and promote End of Life Doula certification to potential interested participants.
I agree that as an End of Life Doula, I will honor the needs to care for myself physically, mentally, emotionally and spiritually, as needed, to maintain health and strength as it pertains to my own life.
I agree as an End of Life Doula, I will remain current and competent in my skills and abilities and to enhance the professional development of End of Life Doula’s in Utah through my certification. Further, I will attend the annual education session and seek out more opportunities for Continuing Education.
I agree as an End of Life Doula to support, encourage and mentor my fellow End of Life Doula’s.
I agree to adhere to this Code of Ethics set forth by the Earthside End of Life Doula Training Program, and to hold Earthside Holistic and its instructors/owners/affiliates harmless in the event of unforeseen complications or erroneous behaviors on my part. I agree that I willfully participated in the Earthside End of Life Doula Certification Program and understand that my instructors have given me the best local training available.
