Your Certification will expire 4 years after the date of issuance. You can purchase this packet to re-certify for 4 more years.
We will send you the re-certification packet via USPS/FedEx/UPS.
Make sure you have verification (proof) that you have served at least 5 families in their end of life needs, or have 80 hours of continuing End of Life education for the past 24 months. If you have questions, please contact the Program Director.
